This questionnaire is designed to get you to think of as many different details about the person as possible. Please answer as many items as you can and remember that THE MORE SPECIFIC OF AN ANSWER YOU CAN PROVIDE, THE MORE USEFUL IT WILL BE. You may be surprised at how useful some of these everyday details really are. Just make sure the information you provide is actual and true. We'll add the funny, but it won't work if the information isn't true.

This questionnaire was designed for those clients who 'blanked' or felt 'on the spot' or 'tongue-tied' when asked about the recipient. Have Fun With It!!! It is designed to get you thinking about that person and their likes, dislikes, activities etc. So, please feel free to add any other details that come to mind as you go through this questionnaire. Any stories you can share will be great!
